“There are more people who show up at urgent cares and ERs for things that really don’t belong there. They belong in a primary care clinic, but because there’s no access, you do the best you can as a patient to get the care that you need.” Just like in Pennsylvania, other states across the country are feeling the impacts of a growing physician shortage and lack of patient access to primary care. It can take over a month to see a primary care provider, and wait times for new patients can be even longer. Long wait times are not just inconvenient for patients, but can result in late diagnoses and prescriptions that have run out months ago. By the time a patient gets in to see their primary care provider, they may have a large list to address during a short appointment slot, putting additional pressure on the provider. It's estimated that full practice authority in Pennsylvania would increase the number of patients seen by NPs by an additional 1,792 patients per week! Full practice authority legislation would reduce the burden currently placed on our healthcare workforce, and would decrease appointment wait times for Pennsylvania patients seeking care.

But rather than making it easier for qualified professionals to practice in underserved communities, Pennsylvania's outdated restrictions make it difficult and expensive for NPs to bring high-quality health care to these local communities. Nurse practitioners can help fill in gaps in primary care delivery across the United States. Rather than tethering nurse practitioners to physicians and forcing them to enter into expensive contracts with physicians, the majority of states now allow nurse practitioners the freedom to practice to the full extent of their training. And it doesn’t compromise quality and may even reduce the cost of several services. The U.S. will face a shortage of up to 86,000 physicians by 2036. Primary care and family medicine are already experiencing the impact of this shortage as fewer physicians are going into primary care. At the same time, the U.S. population is increasing and growing older as people continue to live longer. Between 2013-2019, health care visits delivered by nurse practitioners and physician assistants in a year increased from 14.0% to 25.6%, according to an analysis from Harvard Medical School researchers. Nurse practitioners and physician assistants now account for a quarter of health care visits! Relaxing scope-of-practice regulations can lead to increased check-ups and reduced emergency care needs, particularly in rural settings. A 2018 study found that Medicare enrollees treated by NPs had lower rates of hospital admissions and emergency department visits compared to those treated by physicians. The cost of care from NPs was also found to be 11% to 29% lower! There are also strong indications that the quality of patient outcomes is not reduced when the scope of practice is expanded, according to a 2018 study of rural health clinics in 8 Southeastern states. In 2022, NY passed legislation granting full practice authority to NPs. And the results have been overwhelmingly positive. Nurse practitioners are happy to see their skills being applied in rich and meaningful ways, and doctors are happy for relief from the pressures of relentless demand. “Given the staggering statistics on suicide in our nation, voting to pass Senate Bill 25 and grant nurse practitioners full practice authority is truly a matter of life and death,” said Kirstyn Kameg, PMHNP and co-founder of Peace of Mind Psychiatry. Last week, Senator Camera Bartolotta joined PMHNPs and mental health advocates in Washington County, PA to advocate for Senate Bill 25, which would remove the barriers patients face while trying to access physical and mental health care in Pennsylvania. "Pennsylvania has the opportunity to address this crisis by loosening the restrictions that prevent nurse practitioners from practicing to the full extent of our training and skills," said Jen Bowman, PMHNP and co-founder of Southpointe Psychiatry & Wellness. New research shows states that grant full practice authority to nurse practitioners reduce total healthcare costs for people with diabetes by a whopping 20% in urban areas! And in rural areas, full practice authority drives down usage of advanced medical services by people with diabetes by about 10%. The study also found that full practice authority had no effect on primary care claims, the number of high blood-sugar tests, or diabetic debridements, which are common procedures for people with diabetes. Ending burdensome collaborative agreements and allowing NPs to practice independently would also let doctors reserve more of their time for the complex clinical situations they have been uniquely trained for. There’s no good reason to preserve outmoded scope-of-practice laws that artificially constrain the supply of health care — and hurt patients and drive up costs in the process. Nurse practitioners are highly trained. It’s time for Pennsylvania to let them practice to the full extent of their capabilities.
