Join Our Team at Palolo Chinese Home! We are seeking an Occupational Therapist/Program Manager (Contractor) to support our HUD Older Adult Home Modification grant program! If you’re passionate about making a difference in the community, this is a rewarding opportunity for you. Key Responsibilities: Conduct home visits and develop care plans with a multidisciplinary team. Assist clients in regaining activities of daily living (ADLs) and recommend modifications for safety and accessibility. Oversee operations, project reviews, and coordinate with contractors. Qualifications: Bachelor’s or Master’s degree in Occupational Therapy (ACOTE accredited). NBCOT certification and Hawaii OT license. 5+ years in Environmental Accessibility Assessments or relevant certifications. Submit your proposal by October 31, 2024.
