Patient Safety Authority was established under Pennsylvania Act 13 of 2002, the Medical Care Availability and Reduction of Error (MCARE) Act, as an independent state agency. It operates under an 11-member Board of Directors: Our Mission: Improve the quality of healthcare in Pennsylvania by collecting and analyzing patient safety information, developing solutions to patient safety issues, and sharing this information through collaboration. Our Vision: Safe healthcare for all patients. We will review challenges related to shoulder dystocia cases and evidence-based practices for prevention and response when a shoulder dystocia occurs, as well as steps that can be taken after such an event, to inform practitioners with the aim of improving outcomes for future instances.     Want to learn the best ways to identify risk reduction strategies in your healthcare facility? Patient safety officers of all Pennsylvania healthcare facilities must “ensure the investigation of all reports of serious events and incidents,” which is vital to the identification of patient safety concerns and implementation of risk reductions strategies. During review and analysis of patient safety event reports in the Pennsylvania Patient Safety Reporting System (PA-PSRS), the Patient Safety Authority identified opportunities for improvement related to event investigations as evidenced in the limited information provided and follow-up communication with facilities. To address this issue, PSA conducted a yearlong project to assess current practice at healthcare facilities and provide education to patient safety professionals about event investigation concepts. In addition to the assessment, facilities were offered focused, monthly webinars that culminated in three in-person workshops on various investigative skills and techniques. This toolkit provides a compendium of the webinars, as well as a summary of several key concepts presented to provide support for facilities’ investigations, which will lead to improved reporting; better analysis; and ultimately, more appropriate and effective risk reduction strategies.     Since it was established in 1986, International Infection Prevention Week raises awareness of the importance of infection prevention. From healthcare-associated infections to flu and other infectious illnesses, infection preventionists (IPs) play a crucial role in protecting public health.     Just published: As medical care, social norms, and laws change, it is important to review the process of collecting medical information to ensure that new medications, treatment modalities, and other factors are considered. Instead of asking patients to indicate pertinent medical and surgical history on a paper or electronic form, it may be helpful to actively discuss patients’ history with them. This will allow for both healthcare providers and patients to better understand the risks involved and how changes may be made to their course of treatment.     Mpox, previously known as monkeypox virus, is an infectious disease spread through close contact and respiratory droplets. Exposure to the virus can occur during travel to an area with sustained human-to-human transmission; contact with a confirmed, probable, or suspected case; contact with a dead or live wild animal or exotic pet that is a central African endemic species; or use of a product derived from such animals (e.g., game meat, creams, lotions, powders, etc.).     JUST PUBLISHED: Incorrect administration of blood products is a critical concern, occurring once in every 12,000 units transfused in the United States. Blood product documentation and scanning are vital for patient safety, yet difficulties in scanning multiple barcodes on blood transfusion bags often lead to delays and the need for manual overrides. This quality improvement project utilized the PDCA (Plan-Do-Check-Act) cycle of improvement to address blood product scanning in a 495-bed hospital in New York City. The scanning rate for blood products increased significantly from 69% in January 2021 to 96% in May 2021, representing a 39% improvement. Subsequently, increased compliance in 2022 demonstrated sustainment of best practices. Implementation of the PDCA method led to increased compliance in scanning blood product bags following barcode scanner recalibration. Read more and share: https://lnkd.in/eYWtbD5T
