You are in primary care: Knee pain, Middle aged men or women- its not always degenerative meniscal tears and age related changes in the knee. Think beyond- subchondral insufficiency fracture of the knee treated with rest, whereas physical therapy involving leg exercise is commonly prescribed for meniscal tears. Less common are: tumors, avascular necrosis, bipartite patella and intraosseous ganglion all need intervention. There is role for advanced imaging.

You work in Primary care or in interface clinic, always consider there is progression of undifferentiated arthritis to Inflammatory Arthritis, Hence take a wise decision to provide best experience and care to Individual's.
Interface rheumatology clinic referrals from primary care clinicians with undifferentiated presentations: Its clear if Individual presents with: Hand of feet Joint tenderness (1 Point), AM stiffness > 30 Min (1 Point), High Positive RF (1 Point). At Interface clinic arrange US scan for subclinical synovitis and even if -ve: Still benefit from Rheumatology referral as they might need to be followed up at later stage. If +ve (1 Point). Risk of progression of Inflammatory arthritis: 2 point- 31%, >3 points- 62%. (ten Brinck RM, van Dijk BT, van Steenbergen HW, et al.
