    How does #chiropractic alleviate cervicogenic CNS symptoms? When we consider the interrelationship of normal pressure hydrocephalus, spinal stenosis, and cervical spondylotic myelopathy, the effect and influence of chiropractic spinal adjustments on CSF flow dynamics bears consideration. The paper Tatsumura et al. provides insight into the effect of extradural spinal cord compression and the impedance of CSF flow. While RA instability of the atlantoaxial joint complex is an absolute contraindication to chiropractic care, this case offers evidence regarding the effect of extrinsic obstruction of craniospinal CSF flow and related neurologic deficits.     Chiropractic doctors are experts in spinal biomechanics. Many thanks to Cameron Bearder, DC for sharing his amazing Cone Beam CT images with us. In these upright, lateral flexion 3D CBCT images we can observe beautiful evidence of a patient with normal cervical coupling versus another patient with #chiropractic subluxation/fixation/dysfunction/paradoxical movement (insert your term here). First, consider the cause of that stuck and fixated C2 segment on the right. Is it the result of traumatic disruption of zygapophyseal joints, capsular damage, articular cartilage damage, hemarthrosis, synovitis, synovial hypertrophy, capsular adhesions, loss of segmental motion, DJD, and segmental muscle atrophy? Second, consider the ramifications of that lost intersegmental mobility. Altered proprioception, dysafferentation pain, cord and CNS sequelae? Progressive DJD? Can this produce denticulate ligament tension, epidural membrane pathology, and neurologic compromise? Finally, barring any contraindications, how would you adjust this patient?     Chiropractic is a primary means to alleviate the opioid crisis. Current medical practice guidelines recommend the optimization of initial utilization of spinal manipulation rather than opioids as first-line therapy for acute or chronic non-cancer, musculoskeletal pain. #Chiropractic care has been associated with lower chances of receiving prescription opioids and a reduction of #opioid dosages amongst those already prescribed. In this new protocol paper by Emary et al., the authors report supportive data from many researchers that confirm the benefit of chiropractic spinal adjustments: -- 12% lower odds of incident opioid use. -- 44% lower odds of long-term opioid use. -- 34% lower rate of prescription opioids filled over 12-month follow-up. -- 73% lower opioid refills. -- 78–86% less likely to be prescribed higher opioid doses. The authors report, "These findings, combined with those of other reports suggest that access to chiropractic services may reduce reliance on opioids for acute or chronic musculoskeletal pain." Ancillary medical providers, hospital systems, health care providers, corporations, the Medicare and Medicaid system, and responsible legislators need to prioritize chiropractic as a priority initial point of care for non-cancer neuromusculoskeletal care.     #Chiropractic doctors are masters of segmental motion. Consider degenerative osseous, ligamentous, and IVD intrusions into the spinal canal and neural foramen. Consider the dynamic nature of segmental motion with fluctuating infolding and intrusion that are movement-dependent. Think about the effect of compression on the flimsy walls of the radiculomedullary veins/plexi, and how compression can produce venous insufficiency, stasis, and increased parenchymal pressure.
