This is a bit of a long caption, but we thought it was interesting enough to share at length. In October our research team published a study in Menopause. This research is the first peer-reviewed study to compare FDA-approved patches and gels to compounded estrogen creams. The data confirm similar E2 absorption between low dose patches and gels. We also published data from compounded creams showing that levels increase significantly with therapy, but at lower levels compared to E2 gels. The article also highlights the potential clinical validity and utility of urine metabolite testing in the setting of transdermal estrogen replacement therapy. Dr. Pam Smith wrote a letter to the editor following our publication and her questions captured some common misconceptions about HRT. We were given the opportunity to address these important topics in our response to her letter in Menopause. Many providers share Dr. Smith’s confusion over the naming convention of E2 patches (delivered dose) compared to gels/creams (administered dose). This confusion is compounded by the spuriously elevated levels seen in saliva with gels/creams but not with E2 patches, and we were glad to be able to clarify some of this confusion in our letter. It's exciting to be able to have these discussions in an open forum. From this discourse, it's our hope that we can all become more aware of hormone replacement therapy advancements and how to better support patient needs by following the evidence.
