Our study with the PI-CAI consortium, investigating the noninferiority of biparametric MRI (bpMRI) in comparison to multiparametric MRI (mpMRI) for diagnosing clinically significant prostate cancer (csPCa), has been published in European Urology: https://lnkd.in/eq7TymY7 (open-access) 𝗦𝘁𝘂𝗱𝘆 𝗢𝘃𝗲𝗿𝘃𝗶𝗲𝘄 👩⚕️ In this observer study, we evaluated 400 MRI scans (of mixed imaging quality, see https://lnkd.in/eE9cPAyB for PI-QUAL v2 ratings) from four European tertiary care centers, assessed sequentially by 62 independent radiologists (45 centers, 20 countries; see https://lnkd.in/da_RbV8h for their ESUR/ESUI-based expertise level). The results showed that bpMRI delivered similar diagnostic accuracy to mpMRI, with AUROC values of 0.853 and 0.859, respectively, along with comparable sensitivity and specificity, confirming bpMRI's noninferiority within our prespecified 5% margin. Notably, we found limited additional benefit of DCE MRI for non-expert readers. Decision curve analysis revealed that mpMRI maintains the highest net benefit in cancer-averse scenarios, and that bpMRI maintains the same for biopsy-averse scenarios. 𝗖𝗹𝗶𝗻𝗶𝗰𝗮𝗹 𝗣𝗲𝗿𝘀𝗽𝗲𝗰𝘁𝗶𝘃𝗲 🔎 With large-scale retrospective evidence, we demonstrate that bpMRI provides comparable diagnostic accuracy for csPCa compared to mpMRI, underscoring its potential to streamline prostate MRI. To ensure the safe integration of bpMRI into clinical practice, when applicable, it is crucial to prospectively validate these findings across diverse clinical settings in international trials.
