“Impact” is ProxsysRx’s 30-day readmission reduction program, using a clinical pharmacist in collaboration with a nursing team. It starts at discharge, when the pharmacist conducts a Comprehensive Medication Review bedside — and it continues with a Patient Coordinator / Nurse reaching-out to a patient at least four times over the course of 30 days, post-discharge In the program’s first month, for patients enrolled in the program, readmissions were 55% lower than the health system’s historical rates. In 10 months, the health system saved $486,400 (a figure based on DHS’s estimated average readmission costs), and patient satisfaction among those enrolled was rated 9.77 out of 10. Let’s hear it for HRSA standing-up to the drug manufacturers, for a change! Johnson & Johnson recently informed 340B hospitals that, beginning Oct. 15, it would only offer 340B discounts on top-selling drugs Stelara and Xarelto — prompting nearly 190 bipartisan members of the U.S. House of Representatives to urge HRSA to “use every enforcement tool” at its disposal against J&J’s “unapproved and unlawful” 340B rebate proposal. Sure enough, last Friday the agency sent J&J a letter threatening to terminate its Pharmaceutical Pricing Agreement — which would end Medicaid and Medicare Part B coverage for all J&J drugs — if it didn’t abandon its unapproved plans. Long overdue action, if you asked us — But hey, it’s a start!
