Emma embodies the compassionate and hardworking professionals—nurses, social workers, and community health workers—who work in hospitals and clinics to connect individuals with essential services. Whether it's facilitating access to healthcare for short rehab stays, connecting with community organizations to address social determinants of health, or navigating state benefits like Medicaid, Emma is there. She also explores alternative care delivery methods to ensure no one is left behind. Through engaging animations, Emma walks us through real-life scenarios using our RE-Assist software. RE-Assist had the privilege to engage the panel discussion on Critical Issues and Innovation in Healthcare. There was discussions on private, public, technology, and equity in healthcare. The expert insights shared by leaders from Walgreens, Oracle Health, Northwell Health, Pandemic Response Institute and many others truly expanded our understanding of how to drive health equity and improve care delivery across sectors. We learned that in some states, primary care accounts for only 3% of the Medicaid market. The networking breakfast set the tone for meaningful connections, and the panel discussions that followed sparked inspiring conversations about the future of healthcare innovation. Excited for how RE-Assist can continue to drive on these learnings forward. RE-Assist attended the insightful session on "Community-Driven Health Equity: Harnessing Local Leadership and Innovation" at the Doris Duke Center. The expert panel discussion highlighted the urgent need for collaboration to address health disparities, especially for vulnerable populations. Despite our nation's GDP, funding for these crucial initiatives remains inadequate. Key points included the importance of engaging local leaders—women, children, faith leaders, and community organizations—to understand their unique needs of our community. With the question of what do you need? and how do you want it? Continuous quality improvement and investing in the infrastructure to scale the solution. Some coordinators, manage large caseloads ranging from 50 to 100's of patients and often facilitate a specific subset of services. These coordinators frequently lack relevant real-time patient history and encounter barriers such as unaccounted social determinants of health. They typically need to invest additional hours to connect patients to services, obtain medical necessity forms, handle incident reporting, and more. This often results in working beyond the standard 9-5 schedule, leading to burnout. RE-Assist is a software solution designed to alleviate the administrative burden of sourcing real-time data with an easy to use interface for these coordinators. It prescreens for social determinants of health, provides eligibility requirements / forms, and suggests service providers that can assist before the coordinator meets with the patient, preventing issues like utility shutoffs and worse.
