About us

The RE-Assist application is a care coordination platform that replaces the paper list directory in care planning by integrating the role of the patient, case manager, and service provider. This will lead to reduction of readmissions, length of stay and increasing utilization between providers, productivity and patient satisfaction. On the back end, there is trending, tracking and quantifiable data to identify wasteful and potentially harmful resources.

    Some coordinators, manage large caseloads ranging from 50 to 100's of patients and often facilitate a specific subset of services. These coordinators frequently lack relevant real-time patient history and encounter barriers such as unaccounted social determinants of health. They typically need to invest additional hours to connect patients to services, obtain medical necessity forms, handle incident reporting, and more. This often results in working beyond the standard 9-5 schedule, leading to burnout. RE-Assist is a software solution designed to alleviate the administrative burden of sourcing real-time data with an easy to use interface for these coordinators. It prescreens for social determinants of health, provides eligibility requirements / forms, and suggests service providers that can assist before the coordinator meets with the patient, preventing issues like utility shutoffs and worse.     As the world was hit with unprecedented challenges of COVID-19 in 2021, Aaron Bess Jr., a remote learner with a brilliant mind from Forest Park, emerged not just as an innovator but as a beacon of hope. Spearheading the development of RE-Assist's first MVP in 2021 at age 11, as our Junior Technical Architect, Aaron redefined the boundaries of what young innovators can achieve if given access and opportunity. Celebrated for his exceptional skills, Aaron has been recognized by esteemed leaders like Dr. William Barrett and Dr. Abdul Rahman Jazieh, and honored by the Mayors of Forest Park and Cincinnati. His journey includes prestigious awards, such as being part of the MIT Solve Global Solver team in 2022. Many individuals in the Cincinnati Entrepreneurship Ecosystem have supported his journey including him in accelerators and bootcamps at his young age. Winton Woods City Schools have provided mentorship and encouragement to support his skillset. Aaron's path is a testament to the transformative power of technology, serving as a beacon of inspiration for young individuals.
