HCDI participates in two regional health planning efforts that bring together various stakeholders to address health needs in the region, the Population Health Improvement Program and the Asthma Coalition of the Capital Region. The Population Health Improvement Program (PHIP) is a New York State Department of Health program that supports the development of regional strategies to transform health care delivery, improve population health and eliminate disparities. The program uses regional and local public health data to develop coordinated, evidence-based strategies to measurably improve population health, as well as, provide technical assistance to partners in the development of population health initiatives. The PHIP provides services primarily, but not exclusively in Albany, Rensselaer, Schenectady, Saratoga, Columbia and Greene counties. The Asthma Coalition of the Capital Region (ACCR) is a partnership of medical and public health professionals, universities, foundations, schools and other member organizations, coordinates sustainable initiatives to reduce the burden of asthma in Albany, Rensselaer, Schenectady, Fulton & Montgomery Counties. The Coalition provides resources, education, and tools to support evidence-based asthma care and self-management.
