When I first started this Resilient Healthcare journey, I had no idea how much regulation would play a role in what we do, especially considering everything we provide in patients’ homes is well within our scope of practice. Step one: Is it legal? Step two: How do we get paid for it? Shockingly, it’s often not about what services are being provided, but where they’re being delivered. The place of service can be a regulatory roadblock, making the same care that’s legal in one setting not permissible in another. Navigating these nuances is part of the challenge—and the opportunity—in reshaping home-based care. Did you know a PTA doesn’t need direct supervision when seeing Medicare patients for a home health agency but does when seeing Medicare patients in an outpatient clinic setting? MAKE IT MAKE SENSE. I’ve been seeing a lot of posts from physical therapists on LinkedIn who are either looking to leave clinical care or have already done so—and they’re often preaching about it. My journey was a bit different. I genuinely loved patient care. But as my business grew, my investors at the time asked me to step away from clinical work and take on management full-time. Let’s be real—clinical care just doesn’t pay like it used to. Those days are gone. The thing is, I love treating patients. And I’m sure most physical therapists do too. But the lack of pay and the increasing documentation requirements are burning us out. Here’s the frustrating part: we’re leaving it up to startups (usually underpaid providers who want a change!) to take the risks and try to drive change in our industry, hoping they’ll come up with a new delivery model and deep tech that actually improves the experience for both patients and providers. But somewhere along the way, this $4.1 trillion industry forgot that it was built on the patient-provider relationship. Even though healthcare spending has skyrocketed, patients and corporations AND start ups are paying more, and clinicians are getting paid less. Most startups also have to juggle tight cash flow and operational nightmares, like making sure payroll goes out on time, which just adds to the pressure of whether we can ever really solve this healthcare mess. But nonetheless Resilient is working on something. We’re bringing together big tech, tried-and-true healthcare systems, and a startup mentality to innovate in ways that matter. I can’t wait to share this project with you guys. (Yes, I am a Jersey girl) It’s the result of the last decade of my life—the countless sleepless nights, meetings, whiteboard sessions, investor pitches, conferences, patient encounters, and continuing education. And this is just the starting line. P.S. As of today, I am back to patient care part-time while managing this company and still being a mom who shows up at her kids sports events. That’s what happens when ethical technology meets an industry in need.

On its end, Choice Health at Home has a relationship with Community Hospital Consulting that spans almost 20 years. Even prior to this introduction, through Community Hospital Consulting, leaders at Choice Health at Home had been aware of Resilient’s work. “Jackleen and Resilient did a lot of good work during the pandemic with the initial initiative from CMS, so we’ve watched that from afar … and we were excited to have the opportunity to work with Resilient,” Choice Health at Home CEO David Jackson told HHCN. “It’s one thing to program develop and whiteboard what it might look like to reinvent the delivery of health care, it’s a whole other thing to actually deliver that care,” she said. “As we’ve become pioneers or experts in the space of program development for hospitals to enter the community, working with somebody like Choice Health at Home that has a 20-year relationship in these communities and delivering this care, and knowing the clinical aspects of delivering that care, is the one of the best partnerships that we could have thought of.” Both Samuel and Jackson noted that the partnership between Choice Health at Home and Resilient aims to bridge the gap between home health care and hospital at home.
