“Based on how well health plan members control their diabetes, we have developed new tools that allow the clinician and the member to work together to develop a specific diabetes care plan.” Our chief medical officer Heidi Schwarzwald MD MPH spoke to McKnight's Home Care about our new diabetes Focused Visits, positioned to inform effective treatment decisions and improve care for the growing population of adults with diabetes. During these visits, our clinicians offer screenings, such as diabetic eye examinations, chronic kidney disease testing, and blood sugar measurement, to get a more complete picture of the plan member’s health, provide them with the right information and guidance to manage their condition and connect them with their primary care provider.
I had the pleasure of speaking with @Adam Healy McKnight's Home Care about the recent launch of Signify Health’s Focused Visits. Starting with #diabetes, these Focused Visits are designed to close care gaps and slow disease progression for those with the most complex conditions.
