🌍 World Mental Health Day is a powerful reminder that mental health is not just a personal matter—it's a critical social driver of health that impacts communities, workplaces, and healthcare. When left unaddressed, mental health challenges can lead to higher healthcare utilization and barriers to accessing care. Let's continue working together to eliminate the stigma and shape a healthcare system that prioritizes mental health as essential for equitable, whole-person care. #ThinkSpatially #mentalhealth #healthequity #SDOH

A cloud-based Decision Guiding Platform that operationalizes SDoH to promote health equity.
About us
Spatial health empowers ACOs and provider teams with actionable SDOH insights to address health barriers at the individual level for improved care delivery. Our Equity Equalizer™ Platform is a cloud-based decision-guiding platform that ties Social Determinants of Health (SDOH) factors to health outcomes and costs. We empower healthcare organizations to achieve health equity by providing actionable insights into the health barriers facing their patient populations. Our Equity Equalizer™ Platform leverages spatial analytics, location intelligence, and machine learning to identify and quantify individualized health barriers at the hyper-local level. This goes beyond ZIP codes to provide granular insights into the unique SDOHl factors that impact individual health. With Spatially Health, healthcare organizations can: - Gain comprehensive insights into their patient population's most prevalent health barriers to prioritize outreach and ensure they receive targeted support for improved health outcomes. - Make data-driven decisions about which interventions to prioritize based on patient health factors and intervention effectiveness. This enables provider teams and care managers to allocate resources more effectively and deliver the most appropriate care. - Proactively identify and engage high-risk patients by evaluating the risk of their patient population and understanding the associated costs. Prioritize outreach efforts to the most vulnerable individuals, helping improve health outcomes, reduce readmission rates, and maximize bonuses while avoiding penalties. - Empower provider teams and care managers with comprehensive patient insights that consider medical data alongside health barriers and prioritized intervention recommendations for personalized and effective care. Spatially Health is dedicated to integrating SODH seamlessly into healthcare practice to provide patients with holistic care that addresses the underlying cause of their health challenges.

A recent Deloitte report shows that improving #healthequity could add $2.8 trillion to U.S. GDP by 2040. Currently, direct medical costs linked to health inequities total $320 billion for the healthcare system. By 2040, this number could be $1 trillion if health disparities remain unaddressed. Tackling this issue could prevent 5 million workers from leaving the workforce due to premature death or severe disability. Read more https://hubs.la/Q02RD_6V0 #ThinkSpatially #healthcareinnovation

Value-based care promises better outcomes and reduced costs for patients, yet only 46% of primary care physicians reported receiving any payments from value-based care arrangements. Key challenges to this transition include: 🔸 High administrative burdens 🔸Staffing shortages 🔸Complex performance metrics To turn the vision of value-based care into a reality, the industry needs payer collaboration, simplified processes, and better incentives for providers. A recent study from Cornell University published in Health Affairs titled "Meeting the Needs of Socially Vulnerable Patients: Views of ACO Leaders on Moving From Intent to Action," discusses how ACOs, especially those in the MSSP program, are stepping up to meet the needs of vulnerable patients. The study interviewed 49 ACO leaders, and here’s what stood out: • ACOs often prioritize medical risks over social ones when stratifying patients. • There’s much interest in pilot programs aimed at social issues like isolation and high drug costs. • Rural ACOs are facing some unique hurdles. At Spatially Health, we tackle these challenges head-on with our Equity Equalizer™ platform. It’s designed to make SDOH insights not just informative but actionable. Here’s how we’re helping: • Stratification that Matters: Stratifies patients by social risk, offering a comprehensive view of the population for better intervention targeting. • Practical Solutions: We focus on health-related social needs that directly impact medical care, ensuring provider buy-in. • Streamlined Implementation: We lower the barriers to starting new social needs programs by providing the infrastructure, analytics, and workflows needed to get things up and running efficiently. • Real-Time Performance Tracking: Our dashboards make it easy for ACOs to track their progress on high-priority measures for socially vulnerable patients. At Spatially Health, we're committed to supporting ACOs in their mission to improve care for vulnerable populations.
