Thank you to Jessica Hagen and HIMSS Media for the opportunity to step back and talk about what our recent string of announcements means! It is true that getting more patients on optimized medications for their #heartfailure, faster, was the first application of our new specialty care delivery model. In partnership with our early health system customers, we proved that this new model improved both clinical outcomes and clinic efficiency. But the vision was always to extend this new care model and transform specialty care delivery across the board. In the last twelve months, we’ve added more conditions beyond heart failure (#hypertension and #afib), and rapidly expanded our roster of customers and partners, including #valuebasedcare organizations in addition to #healthsystems. “Story Health: It’s NOT an app.” That’s not our new tagline, though maybe it should be! At #AAHFN2024 last week, the first question many booth visitors asked was “So, are you an app?” One of our guiding principles as an organization is to make our programs as accessible as possible. Regardless of where patients live, how savvy they are with technology, or their socioeconomic status - just to name a few - we prioritize ease of use in the patient experience to help improve access. That’s why we do NOT have an app, and the biometric devices we use connect automagically (marketing term, not a tech term). We can’t help patients manage their health if they can’t or won’t engage with the program. So far, it seems to be working. Two highlights from #AAHFN2024 so far: 1. "Words matter" discussion from Anuradha (Anu) Lala-Trindade (Lala)'s talk about 2024 Heart Failure Updates. Choosing words that are empowering and empathetic instead of those that imply something negative (whether intentional or not), can make all the difference in your patient relationships and in inspiring patients to be more active in their care. 2. "...we have done this by blocking off a clinic spot which allows us to take care of 8-12 patients in that slot where otherwise we would be taking care of 1-2 patients..." - quote by Kismet Rasmusson, DNP, FNP-BC, CHFN, FAHA at Intermountain Health during her talk about how her team is using innovative approaches to improve #GDMT optimization.
