We are thrilled to welcome Together Senior Health to the Switchboard! They are ensuring that the 1 in 9 aged adults with dementia are properly identified, cared for and clinically documented. As your partner, those you care for can participate in a live, RCT backed, evidence based digital health program that is proven to reduce falls by 30%, decrease cost of care $4,300 each year, and improve quality of life for both the patient and their caregivers. Developed in partnership with University of California, San Francisco, Moving Together leverages muscle memory, mindful body awareness, music and meaningful connection to others. Real-time video connection with the same instructor and classmates each week provides “eyes on the member” that enable socialization among small groups of participants and reduces the safety risk associated with other unmonitored therapies. In July 2024, Together Senior Health was acquired by Linus Health. This now enables them to provide the full continuum of services for those living with cognitive decline, from assessment to therapy.
