These individuals possess a deep understanding of the youth and family dynamics, demonstrating genuine care and providing support without financial compensation. They play a vital role in offering long-term assistance, which ultimately allows the wraparound facilitator and other professionals to gradually transition out of their intensive service roles. We are particularly interested in proposals that highlight your community's knowledge, experience, and effective strategies for establishing and managing high-fidelity wraparound in systems of care. The Wraparound process is a team-oriented planning approach designed to deliver personalized, coordinated, family-centered care for children, youth, and families engaged with various child and family-serving systems. This method specifically targets those at risk of being placed in institutional settings and those who may be experiencing emotional, behavioral, safety, or mental health challenges. By bringing together families, service providers, and essential members of the family's social support network, the Wraparound process facilitates collaboration in developing a tailored care plan that addresses the distinct needs of each child and family.
