About us
The Learning Network’s goal is for home-based primary care (HBPC) practices to improve the care provided to homebound older adults, create a culture of quality improvement and continuous learning in and create a community of quality in HBPC. The Learning Network: • Provides support to help HBPC practices implement quality improvement principles and demonstrate high-value care. • Helps practices and clinicians connect and build a community in a practice-based learning network so they can together as to improve care and raise the field of HBPC. • Provides complimentary access to a state-of-the-art quality improvement software package.
