💡 “Patients that are at risk, specifically those that have social determinants of health needs, are the ones that typically have the highest hospitalization rates, the highest rates of ED visits, and the poorest outcomes. So being able to help them and really get paid for the work that we do to help them with their needs is vital for performance in any payment model so that those patients can have access to the highest quality cancer care available,” shares Dr. Lalan Wilfong, M.D. In the AJMC - The American Journal of Managed Care interview, he explores how patient navigation services, especially under new payment models like the Enhancing Oncology Model, can reduce unnecessary hospital visits and improve adherence to therapies. Navigators should be able to provide full-focused support to patients when they need it most, and we need to better understand what high-value services to prioritize. As practices take on more risk, Dr. Wilfong believes patient navigation will play a crucial role in improving overall performance in a total cost-of-care model. Dive deeper into the discussion on patient navigation’s future here: https://lnkd.in/eJtuei7B
