On National Truth and Reconciliation Day Team Primary Care Project Leads, partners and participants are reminded of the vision that grounded our work in ensuring all have access to comprehensive primary care with teams of providers working together to meet the unique and diverse needs of patients. We saw the power of #TRC Action #22 in its full strength demonstrating the value of Traditional Healers, Elders and Knowledge Keepers as fully participating healthcare team members. We are grateful for the deep learning gained as settlers involved in the implementation of Team Primary Care and for hard learnings impacting our own journeys, due to errors made. We are grateful for the deep reflection and heartfelt grace yet firm accountability of our Indigenous Partners and Indigenous Advisory Council. Team Primary Care: Training for Transformation, a project launched in 2022 and funded by Employment and Social Development Canada, brought together more than 100 health professional and educational organizations with the goal of accelerating transformation change in the way primary care practitioners train to work together. Team Primary Care’s goal is to help create a more sustainable system by transforming interprofessional care training in Canada. Team-based care allows for better patient access, as people receive medical care when they need it from the appropriate provider and are actively engaged in their care. Team-based care also increases healthcare capacity, enabling more Canadians, including Indigenous people, the elderly, and those living in rural settings to gain timely access.
