Today is World Homelessness Day and we believe that housing is a human right. More than 6,000 people in Portland people face homelessness every night, and three in five people experiencing homelessness are living unsheltered. Last year, Transition Projects provided 3,154 individuals a safe bed inside at one of our eight shelters — which accounts for one-third of all people sheltered in Multnomah County. On top of that, our team placed 894 people into affordable housing! Helping people secure — and retain — permanent housing is the ultimate goal of our work. There is still plenty of work to do, but it is clear that when we have housing resources, we can increase the number of people transitioning from shelters into housing. Helping people transition from homelessness to housing.
About us
Transition Projects provides individuals with the services, resources and tools they need to end their homelessness, secure housing, and maintain that housing. - Each year, we assist more than 10,000 people experiencing homelessness. - On any given day, we help meet the basic needs of more than 700 people through our Day Center. - On any given night, we provide a safe place to sleep for more than 800 people with nowhere else to turn. - In any given year, we help place more than 800 people into affordable housing - and then support them in retaining that housing. Whenever and wherever we can, we advocate for policies and initiatives that affirm the right of all people to a place they can call home. Want to join our team of passionate changemakers? 🎉 SSVF Funding Renewed! 🎉 We are excited to receive funding once again for the Supportive Services for Veteran Families (SSVF) funded grant through Veterans Affairs. In the last year, our Veterans Services team housed 349 Veteran households (comprised of 607 individuals)! SSVF provides case management and supportive services for low-income veterans and is designed to promote housing stability. These grants have been awarded to select non-profit organizations across the U.S. that assist low-income Veteran families who are residing in or transitioning to permanent housing. The SSVF program began in 2012, and Transition Projects has received the grant each year since. Our team includes case managers along with specialists working on income development, benefits, wellness access, and outreach and engagement. It was a great event that has numerous agencies offering free food, supplies, medical services, resources, and haircuts for our community facing homelessness. Overall, we interacted with several hundred participants and community partners, and our Outreach team was able to engage with 84 individuals experiencing homelessness.

Tom has been volunteering with Transition Projects as a Mailroom Assistant in our Resource Center for almost four years. Each week Tom helps sort and hand out mail to hundreds of participants. The mailroom serves approximately 6,000 individuals annually providing a permanent address for people experiencing homelessness. This essential service allows participants to continue receiving their mail when they are without an address while in-between stable housing. Thank you for being on our team, Tom!
