Today, the Centers for Medicare & Medicaid Services released its report to Congress on the Acute Hospital Care at Home (AHCaH) waiver initiative, providing valuable insights from 11,907 unique patients. Here are some key takeaways: - Improved Outcomes: Patients receiving care at home experienced lower mortality rates and fewer discharges to skilled nursing facilities (SNFs). - High Satisfaction: Both patients and caregivers reported high levels of satisfaction with the care provided under this initiative. In fact, "patients and caregivers generally view care provided through the AHCAH initiative as safe, effective, and a positive experience." However, the report also highlights disparities, indicating that AHCaH patients tend to be predominantly white, urban residents, and less likely to receive Medicaid. This underscores the need for expanded Medicaid coverage for HaH services in many areas. Additionally, the report states that hospital-at-home care does not increase—and may even reduce—Medicare spending. This report reinforces what we know: hospital-at-home models are transforming care by delivering high-quality treatment in the comfort of patients' homes. We remain optimistic that Congress will carefully consider these findings and vote to extend the AHCaH waiver for another five years. Nicole Constant had been inpatient at BMC for about a week when she was asked if she wanted to be part of BMC Hospital at Home and receive hospital-level acute care in the comfort of her own home. “I immediately said ‘yes,’” she says. “I knew nothing about the program at that point. I just knew it would allow me to go home.” For Nicole, going home was a big deal. She had spent the previous three months in the hospital, first in Italy, then in New York, before arriving home and being admitted to BMC. “I went to Rome in April and ended up being rushed to an international hospital for surgery because of my Crohn’s disease,” she explains. “When I finally got back to Boston, I needed another surgery.” It was after this surgery at BMC that she was admitted to Hospital at Home. “From the very start, Hospital at Home was a wonderful experience,” she says. “The paramedic who took me home really put me at ease and explained what to expect. Then the tech guy came in and set up everything I needed in my family room. It was really like having a little hospital right in my house. My son was home at night, but he didn’t have to do anything. The program provided everything I needed, from hand sanitizer and IVs to medicine and food. All we had to do was open the door and they took care of everything.” Nicole had several in-person visits each day with nurses or paramedics and met with her BMC doctors and nurses online. “I had 24-hour access to my team through the tablet. Whenever I pressed the button, I’d get my nurse. I felt so secure knowing I could contact them whenever I needed. I really felt I was getting one-on-one care.” One thing that stood out to Nicole was the kindness of everyone who provided her care. “Every person I interacted with was so compassionate and kind, they all went out of their way to make sure I was comfortable. It was a really great experience.” After about two weeks, Nicole “graduated” from the program and says her Crohn’s is now in remission. “I think I healed faster because I was home,” she says. “I feel so blessed to have been surrounded by wonderful medical professionals who were all so invested in my health. Wonk Alert! Great News for Hospital at Home Care from Centers for Medicare & Medicaid Services! We’re excited to share that the CY2025 Physician Fee Schedule Proposed Rule includes a significant development for hospital at home programs. Centers for Medicare & Medicaid Services has proposed to extend the pause on the frequency restrictions for subsequent inpatient visits conducted via telehealth for an additional year (at page 61631). These restrictions, if implemented, would have limited telehealth visits for subsequent hospital care to just once every three days. Given that most daily physician visits for hospital at home patients are conducted via telehealth, this limitation would have severely impacted the viability of hospital at home programs. We applaud Centers for Medicare & Medicaid Services for recognizing the importance of allowing frequent access to telehealth services and encourage the Agency to consider a permanent removal of these restrictions. We’re thrilled to share important news from the House Committee on Energy and Commerce! Following their markup of H.R. 7623, the Telehealth Modernization Act of 2024, the Committee unanimously voted to advance the bill to the House floor. This legislation includes a vital five-year extension of the Centers for Medicare & Medicaid Services Acute Hospital Care at Home (AHCaH) waiver initiative. We commend the Committee for facilitating greater access to hospital care at home, as more hospitals are successfully applying for the AHCaH waiver. With St. Luke's Health System recently receiving the waiver for three of its hospitals on September 12, 2024, we now have a total of 345 hospitals across 39 states that have adopted this initiative. This means that nearly 80% of states now have at least one hospital participating in the AHCaH program! We look forward to seeing the positive impact of these advancements on patient care. Another inspiring hospital at home leader, Eliza Pippa Shulman, doing what she does best - working tirelessly to meet patients, families and providers exactly where they are: at home.
Last week, Medically Home's chief medical officer, Pippa Shulman sat down for a Q&A with Home Health Care News and shared some insights about her experience advancing hospital at home care including how she came to Medically Home: "I came to this as a primary care provider, but I also was doing home-based medical care. I ran a home-based primary care, home-based post-acute care, a SNF program, and worked for an organization where my goal was to keep my patients out of the hospital. When we were looking to set up a home hospital program, we needed help with logistics and technology. Technology is a bit of a commodity. You can get that from anywhere, but how do I get oxygen in the middle of the night to my patient's bedside? How do I get blood drawn when I need it, and get it transported? Home nurses are a wonderful resource, but many of them only travel with the supplies they need that day, and often can't respond to an emergency situation despite the fact that many of them want to, and so you have to retool an entire system. That's really the idea of Medically Home. How can we support logistics and the coordination of care, so that a physician in the hospital doesn't have to worry about whether the care is going to get there?. It is really allowing for the extension of that hospital's brand, so to speak, right into the home. That's the power of the model, and the missing link for so many that want to scale and build, particularly for smaller or mid-sized health systems." There has been a lot of discussion about the technologies that enable hospital at home care, and the elements that the most frequently cited are remote patient monitoring tools. In practice though, one of the most significant technological keys this mode of care uses is a platform that translates a physicians order into care being delivered in the home. This innovative care model, available in more than 300 hospitals across 37 states, is in jeopardy unless Congress acts before the end of this year to extend the Acute Hospital Care at Home waiver. As the president leading Home Hospital at Mass General Brigham, I have seen firsthand how reliable, high-quality acute care at home transforms patient satisfaction and clinical outcomes. It also eases the capacity burden in traditional hospital facilities. Research shows that both patients and caregivers prefer Home Hospital due to excellent clinical results, better experiences, lower caregiver stress, and shorter lengths of stay, which contribute to quicker recoveries. These benefits extend to our cross functional workforce, who report high job satisfaction.
