  • A 35-year-old male patient was admitted due to massive hemoptysis that had been going on for three days. Approximately 200 cc of hemoptysis not resulting in asphyxia was identified. He also reported that he had been experiencing coughing and left-side chest pain for four to five months. Physical examination findings were normal. Patient’s history revealed that acute tubular necrosis had developed after six months of tuberculosis treatment, followed by the onset of chronic kidney failure. The patient stated that he had been receiving hemodialysis treatment for two years.   • A 6-year-old female was diagnosed with GSD1b at the age of 4 months. She was born to third-degree consanguineous parents at term. Her medical history revealed recurrent infections since she was two months old, which led to frequent hospitalizations. The most recent hospitalization in January 2024 at our center was prompted by elevated lactate levels, incidentally discovered during routine outpatient follow-up. The patient had a blood glucose level of 32 mg/dL, a pH of 7.06, and a serum bicarbonate (HCO3-) level of 7.5 mmol/L with a lactate level of 21 mmol/L, suggestive of severe hypoglycemia and severe metabolic acidosis. Following the first physical examination, no pathological findings were observed except for massive hepatomegaly. Assessment for Ebstein-Barr virus (EBV) came back positive. A burst test was conducted to exclude neutrophil function disorders, which came normal; however, her neutrophil counts were repeatedly under 1.5x103. ❓ What is this suggestive of?   • 🫀Myocardial bridging is a congenital anomaly where a coronary artery dips into the heart muscle before returning to the surface. While it’s usually asymptomatic and benign, it can sometimes cause chest pain, arrhythmias, myocardial infarction, or even sudden death. 🫀In this case report, a patient with persistent chest pain for 6 months was found to have myocardial bridging in two segments of the left anterior descending artery.
