Valley Health Foundation (VHF) is a non-profit leader in community health and healthcare that fundraises to support, innovate, and advocate for better health for all in Santa Clara County. VHF helps patients, families, and dedicated healthcare professionals who serve in public healthcare facilities in Santa Clara County. VHF is the foundation for Santa Clara Valley Medical Center, O’Connor Hospital, and St. Louise Regional Hospital, 14 primary and specialty care health centers, Santa Clara County Public Health, Behavioral Health Services, and Custody Health.   • In 2024, the Trauma Center Team at Santa Clara Valley Medical Center (SCVMC) offered 10 fall prevention programs across Santa Clara County at no cost to residents. Participants completed 8- to 10-week programs, learning about fall risks, developing personal action plans to reduce fear of falling, and addressing fall incidents. They also engaged in exercises to improve strength and balance, learned from physical therapist how to use canes and walkers correctly, and tips to getting up safely after a fall. Thanks to VHF donors who supported the purchase of motion sensor lights, non-slip socks, and pillboxes for participants upon completion of this program. Thanks to the Trauma Center Team at SCVMC and VHF donors for their support to ensure better health for all in Santa Clara County.
