🫀Data shows the use of vMap during complex AF ablation procedures can significantly improve freedom from atrial arrhythmias compared with standard-of-care pulmonary vein isolation. ⬇️ A recent study found that additional AF driver site ablation guided by vMap was associated with an 86.5% freedom from all atrial arrhythmias compared to 48.0% of the control group. 🫀Clinical Case with Dr. Gordon Ho, UCSD: Male patient, 70-79 years old, with EF at 40%, presented for VT ablation. Since July, the patient has had 7 VT events with subsequent ICD shocks. 🔎 While the patient was on the monitor prior to the case starting, he had occasional PVCs that were similar in morphology to the clinical VT. This was mapped in vMap which localized a hotspot in the LVOT inferior to the LCC near the LV summit. The LV was accessed via the retrograde aortic approach. Pacing procedures produced a short run of VT that matched the clinical and PVC morphology; however, it was non-sustained. This ECG was mapped in vMap and localized a hotspot in the LVOT, in the commissure between the LCC and RCC. The patient was then paced into a sustained VT and remained hemodynamically stable. 🎯 Dr. Ho then began to ablate in this commissure area, where the VT eventually terminated and was unable to be induced again after pacing. #epeeps #cardiology #EPLab #clinicalexperience #VT
