Venice Family Clinic is a leader in providing comprehensive, high-quality health care to people 45,000 people in need from the Santa Monica Mountains through the South Bay. The Clinic has a network of locations in Venice, Santa Monica, Mar Vista, Inglewood, Culver City, Redondo Beach, Carson, Gardena and Hawthorne plus mobile clinics and an expansive street medicine program to reach people experiencing homelessness. We provide integrated care by creating a one-stop health system that offers multiple services, often at the same locations and same time as primary care appointments. These services include dental care, substance use treatment, mental health services, vision services, child development services, health education, prescription medications, domestic violence counseling, HIV services, health insurance enrollment and more. Featuring: Remí de la Rocha (AltaMed Health Services), Mitesh Popat (Venice Family Clinic), and Audrey Simons (San Fernando Community Health Center) Moderated by Frank Matricardi, DrPH, MPH, FHFMA These three CHCs have developed innovative and effective partnerships with hospitals, health systems, community, and government organizations in the areas of primary and specialty care, including behavioral health. Among other outcomes, they have reduced hospital Emergency Department visits from CHC patients, and strengthened pediatric access, and streamlined inpatient scheduling. Their programs include partnerships in the areas in the social determinants of health (SDOH) areas of food security and distribution, patient transportation, and housing placement. The steady progress of CHCs in providing comprehensive care to needy populations is a model for ambulatory care delivery in California and nationally. Panelists will outline their work in these areas.

Improving her health seemed daunting and all of the strategies she’d tried on her own had fallen short. Then, she joined the Clinic's new walking groups. Every week, Yasmini joins other patients and community members who are also working on their health and fitness goals on brisk walks at parks and walking and bike paths in neighborhoods near several Venice Family Clinic locations. And now, thanks to the support she’s received through the walking groups and other health education programs, she feels like there’s no limit to what she can achieve. We had a wonderful spring and beginning of summer connecting with our communities! Our outreach and Early Head Start teams were very active in providing information about the Clinic's services at events from the Santa Monica Mountains through the South Bay. They celebrated Earth Day in Hawthorne and at Marvin Elementary School, honored Denim Day at Mira Costa and Carson High Schools and attended the Historic Cinco de Mayo Parade in Venice and a health fair in Manhattan Beach. They were also joined by our Health Insurance Enrollment teams at the annual Arts and Literacy Fair in Santa Monica and the Gardena Health Fair. Look for us at similar events all summer long in your neighborhood!
