    In my latest podcast with Dr. Stephen McNeill, we explored why certain low-cost, effective treatments—like niacin—don’t receive the attention they deserve in mainstream medicine. Dr. McNeill pointed out that it’s often a mix of ignorance and greed within the system. He explained how skepticism and attempts to discredit treatments sometimes come from a place of grief over missed opportunities for patients, but also from profit motives that overshadow patient wellness.     Work life balance is important and burnout is very real BUT … When we dedicate ourselves to becoming physicians we are following a higher calling and those entering the profession should understand that we are dedicating our lives to that calling Many of the great advances in Medicine came from those who sacrificed personal time for the greater societal good. It’s a complicated topic and dramatic changes in the physician mindset have occurred because of the corporate takeover of our industry, and this has had negative impacts with a significant loss of physician autonomy Baby Boomers and my Gen X group have witnessed that radical mindset shift in medicine over our careers and tend to see things much differently than the Millennials or Gen Z ….     In this episode of Wellness at the Speed of Light , we welcome Stephen D. McConnell, BS, MSc-CCP, CIS—a seasoned expert who transitioned from performing life-support and anesthesia during open-heart procedures to a focus on preventive care. Over the past two decades, Stephen has established lipid clinics and developed cost-effective protocols for managing chronic kidney disease (CKD) and cardiovascular health. He discusses the shortcomings in current CKD diagnosis and treatment, emphasizing the limitations of relying solely on LDL levels. Stephen advocates for alternative approaches, such as Apolipoprotein B (ApoB) assessment and niacin therapy, which have demonstrated significant reductions in myocardial infarction and stroke rates. Stephen also raises important concerns about the limitations of traditional diagnostic tools like LDL in fully assessing CKD risk. He highlights how LDL measurements often plateau and fail to capture the true risk associated with smaller, more harmful particles—a gap that can be bridged with Apolipoprotein B (ApoB) assessment, which offers a clearer picture of cardiovascular risk. Additionally, Stephen questions the reliance on statin therapies, pointing out that while they are often doubled in dosage, they yield minimal reduction in absolute risk and even trigger increased PCSK9 levels, necessitating further treatment with injectables. By contrast, niacin, which was one of the first successful treatments, shows a 47% reduction in myocardial infarctions and a 24% reduction in stroke, making it a powerful yet often overlooked tool in cardiovascular care.     Mitochondria, often called the powerhouses of the cell, are surprisingly sensitive to a lot of things we face daily. Stress, inflammation, and poor diet can all wear down our mitochondria and make it harder for them to do their job. Even environmental toxins and overexertion add to their workload. Imagine these tiny structures getting hit with too much calcium—it’s almost like breaching their defenses. When mitochondria experience shear—meaning stress that weakens their outer wall—it allows calcium to rush in, overwhelming these tiny powerhouses. Picture a scratch in the cell wall with calcium flooding in from outside, damaging the mitochondria. And when this happens across trillions of mitochondria, especially in neurons, it disrupts cellular function and impacts our brain health.
