Our mission is to guide individuals through the journey of finding and securing safe, stable homes. We provide essential services including financial assistance for initial housing costs and ongoing support to maintain stable living situations. Additionally, we offer temporary residential care for those with medical or behavioral health needs but without stable housing. Employing a scattered site housing model, Rooted Life ensures each residence is fully furnished, creating environments conducive to healing and recovery. Our commitment to these causes has established us as a beacon of hope and support in the community. Rooted Life focuses on progression, foundation and connection. The Role

  • Hours: 8:00am to 5:00pm (Pacific Time, GMT -8:00)

What You’ll Be Doing

You will work closely with referrals to assess their needs and ensure a smooth transition into our various programs. This position plays a critical role in helping individuals access the support they need and conducting comprehensive assessments to determine eligibility and fit for our services.

  • Customer Service Excellence: Providing the highest level of service to both internal and external customers in a fast-paced, team-oriented environment.
  • Active Assessment Calls: Making the assigned number of assessment calls and collaborating with the operations manager to reach team goals.
  • Thorough Assessments: Conducting detailed assessments over the phone to evaluate if the client is a good fit for our services and to identify their specific needs.
  • Service Determination: Determining the appropriate services required based on assessment outcomes and client needs.
  • Client Connection: Ensuring clients are connected to the appropriate staff and services necessary to meet their needs.
  • Seamless Referral Collaboration: Collaborating with other staff and community service providers to ensure seamless referral services.
  • Efficient Prioritization: Prioritizing and working efficiently throughout the workday with strong attention to detail.

Ideal Profile

  • Experienced Social Worker/LCSW/Mental Health professional/Medical Professional with US/Canadian patients
  • You demonstrate exceptional analytical skills, capable of dissecting complex data and transforming it into actionable insights.
  • You pay strong attention to detail and deliver work that is of a high standard

  • You are a self-starter and demonstrate a high level of resilience
