Author Health seeks a behavioral health nurse practitioner to provide clinical care on a multidisciplinary team providing virtual-first care to seniors with serious mental illness (SMI).
WHAT IS YOUR SUPERPOWER?
Perform virtual initial psychiatric assessments, and follow-up care, to seniors with serious mental illness, through collaboration with a psychiatrist
Create treatment plans that may involve prescribing medication, referring to psychotherapy or other modalities of treatment, and determining whether referrals to higher levels of care are warranted
Work collaboratively as part of a multidisciplinary team to address patients physical, behavioral health and social needs, including psychiatry (MD/DO/NP), primary care, social work, community health workers and care coordinators
Deliver patient care and patient education in a manner that is appropriate to a patient s age, physical ability, educational level, and culture, potentially involving the patient s family members in this process
Participate in the coordination of care with PCPs and other medical professionals who are taking care of patients
Be comfortable seeing patients when they need to be seen and doing psychiatric urgent care
When necessary, order laboratory and imaging workup
Timely and comprehensive documentation of clinical work in the electronic health record
Perform similar or related duties as assigned or required
Comfort working within a new care delivery paradigm and fast-paced environment
