The Care Coordination Nurse provides assessment, planning, facilitation and advocacy for optimal clinical and financial outcomes. In collaboration with the attending practitioner, the family and other members of the health care team, coordinates the individualized plan of care and communicates the plan to appropriate stakeholders. The Care Coordination Nurse assists with appropriate use of resources both within the facility and external to the facility. Communication and collaboration may include coordinating admissions with referring facilities and providers, utilization review, discharge planning, transportation and care coordination. The Care Coordination Nurse ensures that high quality care is provided as efficiently and cost effectively as possible.

BSN required
Experience
Five years of nursing experience required. At least one year in pediatric nursing is highly preferred. Nursing experience may include inpatient nursing, case management, utilization review, discharge planning, community health nursing. Utilization review, and/or discharge planning experience desirable.
Pediatric certification, or other Case Management/Care Coordination certification preferred. In some instances, a certification may be required within a specified timeframe.
