JOB DESCRIPTION:  Case Manager                         STATUS:  Salaried - Exempt

SUPERVISOR:  Executive Director                            HOURS:  35 - 40 hours per week        

REVISED: 8/24                                                       SALARY RANGE:  $18 to $20 per hour

ESSENTIAL FUNCTIONS

The Case Manager develops and implements a plan for guests to move to stable housing and financial and personal stability and success. Provides casework and supportive services to families, including related data collection and reporting for guests in all programs. This position requires some evening and weekend hours.

 

Direct Services

·        Interviews with potential guests and maintains waiting list

·        Conducts intakes, including needs assessment and guest history with new guests

·        Creates and helps guests complete goal plan

·        Helps families to utilize community resources

·        Assigns and supervises chores of guests for the Day Center

·        Assists families in their relocation to Congregations.

·        Computes and monitors income information and financial situation of guests; provides budgeting and money-management education as needed

·        Collects guest savings and handles them according to procedures

·        Promotes guest self-esteem and encourages empowerment of guests

·        Meets regularly with guests individually to discuss goals, life skills and other issues

·        Conducts house meeting of all guests at least monthly

·        Provides crisis intervention, conflict resolution, case management and support services for guests

·        Assists with educational programs and holiday activities, including gift distribution

·        Monitors guest compliance with program rules and regulations

·        Provides follow-up services to guests who complete the program

·        Updates church host coordinator with face sheet on each family

·        Maintain and develop relationships with Landlords

 

Advocacy and Referral

·        Provides information and referral assistance to all guests

·        Advocates on behalf of guests

·        Develops and shares resources for guests/staff

·        Maintains contact with referring agencies

 

Recordkeeping and Communications

·        Maintains accurate and up-to-date case notes and files

·        Maintains all pertinent paperwork relating to guests

·        Submits accurate and timely reports as required

·        Immediately informs executive director of serious problems and concerns with guests

·        Maintains confidentiality of staff, guests, volunteers and all program participants

·        Keeps up-to-date records in all required databases

·        Represents the program and its constituents in a positive manner within the community

 

Program Operations

·        Assists with donations, furniture moving, and maintain supply distribution

·        Performs light maintenance and cleaning duties

·        Responds using proper procedures to fire alarms

·        Answers the telephone and directs calls appropriately

 

Miscellaneous

·        Uses personal vehicle to complete job duties including, but not limited to, donation pick-up, outreach, and off-site meetings

·        Assists with other shelter operations

·        Performs other duties as assigned

Working Conditions

Hours: Full time; salaried - 35 to 40 hours per week; flexible

Site:    1346 W. Hamilton Street

Other:  Work at other sites, including congregations, required

Some evenings and weekends required

 

Qualifications

·        Bi-lingual preferred

·        Experience with direct service in a non-profit environment

  • Knowledge of local community resources, agencies and organizations

·        Self-motivated team player who can interact productively and positively with others, but also be able to work independently

·        Strong organizational skills, detail-oriented, self-starter, and self-directed work habits

·        Commitment to the welfare and well-being of children and families

  • Ability to foster relationships with diverse faith traditions
  • Flexibility, adaptability, and creative problem-solving skills
  • Ability to communicate well with all ages, written and orally
  • Proficiency in Microsoft Office applications
  • Ability to organize and keep accurate records
  • Ability to lift 50 lbs. and navigate steps

·        Criminal Record Check required

  • Child Abuse History Clearance required
  • FBI Background Check required
  • Valid Driver’s License and good driving record
  • Personal vehicle or access to independent transportation
  • Proof of motor vehicle insurance
