JOB TITLE: Case Navigator

DEPARTMENT: Family Services

STATUS: Full time, Non-Exempt

PROGRAM: Prevention & Aftercare

REPORTS TO: Family Services Director

TERM: 12 months

HOURS PER WEEK: 40

Position Summary

Under the supervision of the Family & Community Services Director, the Case Navigator will be providing community directed activities that increase Protective Factors in family systems. They will work with clients to increase the economic opportunities, reduce isolation and increase parent knowledge of child development. Case navigators will also participate/coordinate outreach events for the community in collaboration with other community-based organizations and promote programming through social media.

Duties And Responsibilities

  • Will be the point-of-entry via telephone and email contact for community members searching for services within the agency.
  • Will manage communication system (e.g., via telephone, electronic communication, etc.) between community members, clients and agency employees in order to facilitate linkage to services and/or to answer questions.
  • Will manage social media outreach strategies and create program outreach material such as flyers, etc.
  • Will manage a case load of 30+ clients per year to meet contractual requirements.
  • Will provide community classes and workshops in person and through virtual platforms such as Zoom on various subject matter; must be comfortable speaking in public in person and via telehealth methods.
  • Attend meetings and trainings as required by contract and agency. Continue professional growth by participating in classes, workshops and/or conferences, as the budget allows.
  • Conduct intake and assessments for families.
  • Review assessment and screening information with parents, develop action plans, program goals and review progress made on a quarterly basis or as required by program. Will support the families with their achievement towards their goals.
  • Conduct, coordinate and maintain a semi-annual community/ family events.
  • Provide support, comfort, resources and linkages to families in the program.
  • Establish rapport, trust and a professional relationship with coworkers, supervisor, community and stakeholders.
  • Complete monthly program reports.
  • Participate in supervision and administrative meetings held by supervisors.
  • Maintain Quality Assurance/Continuous Quality Improvement standards as per contract.
  • Establish and maintain a current and accurate record keeping system, within program procedures, with adequate provisions for confidentiality. Ensure all files are up to date and accurate.
  • Collaborate and maintain good relationship with other agencies.
  • Report to work at scheduled times, be open to flexible hours to meet the needs of the client and maintain good attendance.
  • Assist with the completion of monthly reports.
  • Serve as the point of contact for the Family and Community Services Department.
  • Other duties as assigned.


Knowledge, Skills And Abilities

  • Knowledge of social entrepreneurship and/other economic development strategies.
  • Proven competent skills, as well as, the ability to exercise independent and good judgment.
  • Good verbal and written communication skills and organizing skills.
  • Ability to follow policies, procedures, and organization.
  • Ability to meet attendance standards and work additional hours as necessary.
  • Excellent sills related to utilizing technology and virtual platforms for service delivery when required.
  • Maintain high level of confidentiality.
  • Develop and maintain effective working relationships.
  • Bilingual (English/Spanish) highly desirable.


Education And Experience

  • BA/BS in Business, Social Work, Human Services, or in related field. AA Degree with applicable experience will be considered.
  • Two (2) to (3) years’ experience in economic development, social services, or a family support program working with at-risk or high-risk families with children under the age of five years.
  • Experience working with culturally diverse families; and have knowledge of community resources available for families of young children.


Licensing/Certification Requirements

  • Some driving is required. Must provide own vehicle, valid California driver’s license and proof of liability insurance.
  • Negative tuberculosis screen (required).
  • Must pass criminal background check.

  • The Case Navigator’s work location is at the Bell Gardens, CA offices with weekly travel throughout the region; the location has moderate noise levels and is a non-smoking environment.
  • The majority of the job duties are performed onsite, however, home visits may be required; employee must climb up one flight of stairs and must occasionally lift and/or move up to 25 pounds.
