Schedule: 9am-5pm, in-hospital only, no home visits
Hours Per Week: 35.00
Hours Per Day: 8.00
Days Per Week: 5.00
Duration: 8 weeks of contract
Shift Timings: 9am-5pm
Responsibilities
Conduct interviews and make initial assessments and referrals.
Manage cases of social service recipients and provide necessary support.
Investigate and verify information obtained from clients.
Provide health-related and social services to clients.
