Am I A Good Fit?
Manage office THL program administrative tasks including management of office THL Client Roster, MCO portals, Health Link reports, Care Coordination Tool (CCT), Carelogic, and billing spreadsheets. Outreach to THL attributed members to provide education about THL program and assist with program enrollment. Provide best practice, quality case management and care coordination services, ensure that all documentation is completed per Payer, State, COA, and HCA’s standards and established guidelines, and assist in marketing new referrals.

What Will I Be Doing?
- Manage and maintain THL Client Roster for the office including but not limited to: assigning Health Link clients to Health Link Coordinators, monitoring CANS/ANSA and care plans for timely updates, monitoring for timely mental health appointments and face-to-face visits, etc
- Outreach to THL attributed members to include program education, scheduling of assessment for program enrollment, MCO portal opt-outs, and documentation of outreach efforts
- Manage office health link census in CareLogic including admissions and discharges to ensure CareLogic census and office UR census match
- Assist Program Director and Clinical Supervisor with Payer audits quarterly
- Complete Health Link peer audits on minimum of 10% of all office Health Link census monthly including submitting to staff for chart corrections and two-week reaudits
- Complete all billing spreadsheets for office and resolve any billing issues for Health Link program
- Maintain all MCO portals for office Health Link clients including attestation, switches to new health home, opt-outs, and discharges
- Complete MCO ADT spreadsheets including client contacts, documentation in CareLogic, and portal updates
- Ensure all Health Link clients on your office’s THL Client Roster are enrolled in the MCO Health Link Portal and data entered correctly in CareLogic; as well as obtain/maintain necessary signatures for the authorization for Health Link Service documents
- Perform other duties as assigned to support operations and mission of the Company
- Bachelor’s Degree in any discipline required. Bachelor’s Degree in human services discipline preferred, including, but not limited to: Social Work, Psychology, Sociology, Counseling, Criminal Justice, etc
- Experience working with children and families in case management type/ community resource position
