Responsibilities
Provide primary care services in post-acute care facilities, including skilled nursing and assisted living
Drive high-quality, safe, and cost-effective patient care
Collaborate with interdisciplinary teams and follow CMS guidelines
Work closely with hospice, palliative care, and home care agencies for safe patient transitions
Provide oversight, guidance, and mentorship to clinicians within facilities
Opportunity Overview:
Schedule: Monday through Friday
Patient Volume: 15-20 patients per day
Compensation: our compensation is a productivity-based model (fee for service) that offers you control to maximize your income potential; expected compensation is estimated salary range of $171,000 to $277,000 annually; this full-time role is benefit eligible
Qualifications:
Preferred physicians with a background in internal medicine (IM) or family medicine (FM)
Physicians with other background who have a passion to get into post-acute care are welcome to apply
