Responsibilities
Collaborate in developing and implementing clinical procedures and guidelines as needed.
Actively participate in staff meetings and case presentations.
Provide supervision to Care Coordinators during initial intakes.
Offer supervision and consultation to staff and student interns when appropriate.
Deliver patient-centered therapy interventions with a focus on patient engagement within the treatment team.
Utilize electronic medical records for documenting patient screenings, assessments, progress, and referrals within agency timelines.
Advocate for patients' access to community resources.
Co-develop treatment plans incorporating input from patients, families, and other agencies.
Coordinate and communicate effectively with the treatment team to optimize patient care.
Use various interventions including group, individual, and family therapy to help patients achieve their goals.
Demonstrate sensitivity to diversity and uphold patients' dignity and worth throughout treatment.
Monitor patient progress and address discharge-related issues, facilitating outside referrals as needed.
Work independently and collaboratively in a dynamic environment, adjusting priorities as necessary.
