Job Description
The Licensed Clinical Social Worker (LCSW) plays a crucial role in delivering high-quality behavioral healthcare services to patients. Responsibilities include conducting psychosocial assessments, providing interventions, making referrals, and consulting with healthcare providers.
Key Responsibilities
Prioritize, schedule, and conduct brief evidence-based therapy for patients experiencing mild to moderate behavioral health symptoms and health behavior issues.
Assess patient needs, make referrals, and advocate for services beyond the brief therapy model when necessary.
Provide crisis intervention support as needed.
Educate and support patients and their families to enhance compliance with medical and behavioral treatment plans.
Collaborate effectively with community partners to ensure comprehensive patient care.
Consult with primary care staff and other providers on patient behavioral health issues, offering guidance on making reports and contacting authorities when warranted.
Maintain thorough and timely electronic medical records of patient visits and related information.
Develop and deliver training sessions, in-services, and updates to providers and staff on behavioral health topics.
Prepare documentation, including diagnostic summaries and treatment plans, as required.
Demonstrate proficiency in the current Diagnostic and Statistical Manual of Mental Disorders (DSM) for accurate diagnosis and treatment planning.
Possess knowledge of psychotropic medications, as well as physical health conditions and chronic diseases.
Apply crisis intervention techniques and adhere to applicable laws and ethical considerations concerning patient rights, treatment, safety, and confidentiality.
This role requires a compassionate and knowledgeable LCSW dedicated to improving patient outcomes through comprehensive behavioral healthcare services.
