The Opportunity
The Medical Assistant (Remote) plays a crucial role at a leading healthcare provider, serving as the core of our care team dedicated to geriatric care for senior living residents. In this pivotal position, you will forge deep connections with patients and their families, ensuring exceptional health outcomes through effective communication and coordination. You will collaborate closely with Primary Care Providers (PCPs), Mobile Phlebotomists, and Clinical Team Leads (CTLs) to deliver comprehensive care. A significant aspect of your role will involve liaising with community staff, integrating all facets of patient care seamlessly.
Hours: Typical Monday to Friday, 8:30am-5pm PST (Full-time/Part-time)
Location: Remote
What You'll Do
Coordinate care with community partners, patients, and families
Collaborate with providers, remote primary care teams, nurses, vendors, and internal stakeholders
Log patient activities and manage Chronic Care Management
Utilize Population Health Tools
Onboard new patients and prepare charts
Handle STAT tasks and routine communications via email, phone, and chat
Undertake special projects as assigned
What We're Looking For
Consistent task completion rate of at least 95%
Proven ability to proactively and accurately manage patient care documentation
Maintain a Net Promoter Score (NPS) of over 80 with all patient interactions
Qualifications And Requirements
High School Diploma or equivalent; some college education preferred
Basic clinical and back-office experience in a primary care setting
Certification from a recognized Medical Assistant Certifying Organization (preferred)
Compliance with Federal Health Care Programs and good standing with government regulators
Remote work setup ensuring HIPAA compliance and confidentiality of patient information
