Outpatient Nursing Case Manager

Description

This position involves running a Medication Clinic and acting as a case manager, with a focus on actively communicating with clinical doctors and customer relations specialists to ensure our clients receive the best care possible. The role requires a Monday through Thursday schedule with a Saturday rotation, and travel to Eastern locations may be necessary for nurse coverage.

Sign-On Bonus

  • $1,500 for Medical Assistants
  • Additional $2,500 for Nurses

Responsibilities

  • Plan, organize, and coordinate the medication clinic.
  • Cross-train to assist in other clinics and regions, with travel coordinated by the Supervisor.
  • Review medication education forms with clients and assess their need for medication refills.
  • Complete all required documentation regarding the medication clinic and psychiatric consultations for EHR.
  • Administer IM medication, dispense samples, and maintain accurate logs on all medications received and dispensed.
  • Assist with prior authorization, maintain injection lists, verify orders, and discuss any side effects with clients.
  • Conduct pre-visit nursing assessments, including taking patient histories and vital signs, and document in patient charts.
  • Complete metabolic screens, medication possession ratio checks, and other quality interventions, documenting in charts.
  • Perform venipunctures and other procedures as directed.
  • Follow medication refill protocols and document accordingly.
  • Screen telephone callers for appropriate referrals and relay messages as needed.
  • Maintain patients' files, records, and other information.
  • Adhere to standard precautions and maintain cleanliness of unit facilities and equipment.
  • Monitor and order medical supplies and laboratory needs as necessary.

Qualifications

  • High School diploma or GED required.
  • Practical Nursing Certificate or Associate/Bachelor's degree in nursing required.

Work Experience/Training/Additional Requirements

  • At least two (2) years of equivalent work experience required if an associate’s degree or medical assistant certification is not obtained.
  • Preferred two (2) years of medical office experience.
  • Knowledge of medical office terminology, procedures, and equipment preferred.
  • Familiarity with Microsoft Office Suite preferred.

Licensure/Certification

  • Medical Assistant Certification required if at least two years of college or equivalent work experience is not obtained.
  • If Licensed Practical Nurse, a license issued in Missouri is required.
  • LPN, RN, or GPN/GN must pass the MO NCLEX exam within 90 days of graduation.
