Arosa seeks an RN Care Manager who will be pivotal in expanding our Company and supporting our Clients.


RN Care Management & Professional Outreach:

  • Work directly with clients and their family members; provide support for home care services and community-based care management services. Conduct professional assessments to understand our client's physical, emotional, social, and spiritual needs, develop a care plan that affords stability, clarity, and peace of mind, and manage the plan. Coordinate with the Interdisciplinary Team (IDT) and oversee Home Health Aide (HHA) recruitment.
  • Be our face in the community: Participate on local committees, task forces, and boards. Tasks may include educating/presenting to professional groups, raising consciousness, and helping professionals learn about how we can help them help their clients/patients, setting up meetings with professionals, getting to know them, figuring out what they need/want, and helping create solutions.

It takes a special professional who is as comfortable with older clients and their families as they are with our client’s satellite support providers, such as Trust Officers, Attorneys, Physicians, and other elite professional service providers. The Ideal Candidate will possess strong relationships with local healthcare professionals and referral sources. They will proactively engage with these stakeholders to expand our client base. Knowledge and Skill Requirements:

  • Educational Minimum Requirement: a Registered Nurse
  • Ability to interface effectively and professionally with client support, including family, physicians, attorneys, etc.
  • Extensive community outreach and clinical leadership are also needed.
  • Ability to work in a team environment required.
  • Preferred: A minimum of two (2) years experience in RN Care Management, with experience working with the aging population, is preferred; other professional experience may substitute.
  • Excellent written and verbal communication skills required.
  • PC literacy required.
  • Valid driver’s license and proof of current auto insurance required.

Salary range: $75k to 90k
