As a Nurse Care Manager, you will play a crucial role in our integrated care team, delivering direct clinical and medical services to individuals in our Health Care Home program with a focus on a "whole person" approach.
Position Details
Full-time, benefit-eligible position
Schedule: Monday-Friday
Responsibilities
Manage a case load of up to 250 Healthcare Home patients
Develop yearly care plans for assigned clients
Assist in creating treatment plan healthcare goals for individuals with chronic diseases
Educate clients on medical needs and provide necessary monitoring
Collaborate with medical providers on significant health information
Educate community and office-based staff on health conditions
Monitor HIT tools and reports for treatment, medication alerts, and hospital admissions
Review monthly reports and collaborate with providers, clinicians, and patients
Conduct Medication Reconciliations for discharged HCH enrollees
Support in UDS measure documentation/tracking/reporting
Work as part of a multi-disciplinary team
Seek culturally competent services for agency consumers
Report to Regional Primary Care Nursing Director for clinical supervision
Perform other assigned duties
Requirements
High School/GED required, Associate or Bachelor's degree in nursing preferred
Proficiency in Microsoft Office Suite and Electronic Medical Records preferred
Registered Nurse (RN) license in MO required
