Primary Duties And Responsibilities

  • Provide remote support to patients in our program through our online platform during shifts.
  • Cover weekend shifts (apprx 8hrs/day based on the real patient case load, coverage time span 9 am to 9 pm PST), provide extra coverage during the weekday as needed.
  • This role requires you to be tech-savvy , and you must be comfortable with working with our internal messaging (Google Hangout, Zoom, Google spreadsheet, pdf, printers, etc. ). Also requires you to be patient and able to explain technology to a non-tech savvy population. You will be required to teach our elderly patients on how the mobile app and medical device works, as well as helping them with troubleshooting with any tech issues.
  • Collaboratively develop an individualized care plan, behavior goals and learning objectives with each patient. Provide individual case management to patients and keep documents with HIPAA compliance.
  • Use the established Standards of Practice and Professional Performance (SOP/SOPP) in diabetes care and evidence-based practice guidelines to determine the educational interventions for each individual.
  • Collaborate with primary care physicians and other clinicians while utilizing the iHealth Unified Care platform and devices designed to track and monitor outcomes and provide educational interventions.
  • Work with a cross-functional product team to develop and constantly improve digital health solutions to extend chronic disease management beyond the doctor's office.
  • May have an opportunity to serve as the leader of a clinical service team, to develop clinical protocols and educational content for our diabetes and hypertension self-management education program that meets AADE and AHA standards.
  • Other duties and projects as assigned.

License/Certifications Required

RD/RDN (Registered Dietitian/Nutritionist)

Qualifications And Skills

  • Excellent communication and patient care
  • Excellent trainability and flexibility
  • Experience working with patients with Diabetes and Hypertension in a clinical setting
  • Ability to perform standard clinical procedures according to established protocols
  • Ability to educate patients and/or families as to the nature of disease and to provide instruction on proper care and treatment
  • Ability to maintain quality, safety, and/or infection control standards
  • Must be a good team player or even a team leader
  • Must have basic good computer skills
  • Good problem-solving ability
  • Bilingual is a plus, especially Spanish, Cantonese, Mandarin, Tagalog, Vietnamese, etc.
