Job Overview

We are looking for a dedicated individual passionate about enhancing patient care and strengthening the connection between patients and healthcare providers. This role involves remote collaboration with patients and medical practice staff, requiring some travel to client locations within the Reading, Harrisburg, and Allentown area (up to 10%).

Key Responsibilities

  • Remote Collaboration: Engage with patients and medical staff from home while working with the Case Management team to manage a targeted patient group.
  • Patient Engagement: Assist patients in navigating their healthcare, addressing inquiries, and connecting them with necessary resources. Help implement effective care plans and address care gaps.
  • Documentation: Keep accurate records of patient medical histories and issues in the electronic health record (EHR).
  • Referral and Authorization: Understand and assist with referral and prior authorization processes.
  • Relationship Building: Develop and maintain professional relationships with medical practices to support positive patient outcomes and reduce medical expenses.
  • Communication: Utilize various communication methods (calls, texts, emails) to engage high-risk patients and escalate medical concerns when needed.
  • Travel: Occasionally visit primary care physician offices as required.
  • Security: Adhere to all policies and procedures to protect patient information and organizational property.

Qualifications

  • Certification: Must be a Certified or Registered Medical Assistant.
  • Patient Coordination: Strong passion for coordinating care for patients with complex medical and social needs.
  • Empathy and Skills: Excellent interpersonal skills, including motivational interviewing, critical listening, and behavioral change management.
  • Experience: Knowledge of medical terminology and comprehension of primary care practices.
  • Bilingual Skills: Spanish language proficiency is an asset.
  • Technical Skills: Familiarity with EHR systems (e.g., EPIC, Athena, Cerner) and various software applications.
  • Attention to Detail: Capable of managing multiple tasks and priorities with precision.
  • Remote Work: Proven ability to succeed in a remote work environment.

Compensation
