Position Type: Full-time (Monday-Friday, possible weekend and evening shifts)
Express Healthcare Professionals is currently seeking a dedicated RN Case Manager for a local Hospice Care company in Honolulu. This role requires a compassionate and skilled nurse to provide exceptional care to patients and their families, both in-home and via telehealth. For more information please call, 808-792-7000.
In-Home and Telehealth Support: Directly provide in-home support to patients, families, and the palliative care team of community-based physicians and nurse practitioners. Offer telehealth (video and audio) support when appropriate.
Patient Assessment: Conduct telehealth patient assessments prior to initial visits and between subsequent visits. Assess patients' physical and mental health, including reactions to illnesses and treatment regimens.
Home Visits & Monitoring: Provide home visits and telehealth monitoring as needed to support the Integrated Care team. Monitor patient acuity to ensure they receive the right care at the right time and place.
Chronic Care Coordination: Identify and coordinate the needs of chronically ill patients and families, supporting clinicians to operate as efficiently as possible.
Electronic Records: Record and report the results of nursing assessments in the electronic medical record system.
Collaborative Care Planning: Collaborate with other healthcare providers to develop patient and family-centered nursing care plans. Ensure the implementation of these plans is well-coordinated.
Treatment Implementation: Implement prescribed treatment and pharmaceutical regimens as authorized by State law.
Patient Education & Counseling: Provide teaching and counseling on health, disease progression, and symptom management.
Care Documentation: Report and document the care plan, nursing care provided, and patients' responses to care.
Safety & Effectiveness: Ensure the maintenance of safe and effective nursing care, whether delivered directly or indirectly (by unlicensed staff, patients, and families).
Advance Care Planning: Facilitate discussions on advance care planning.
Requirements:
Bachelor of Science in Nursing (BSN) from an accredited school (preferred).
Valid Hawaii State RN license.
Minimum Of 2 Years Of Clinical RN Experience.
Current CPR/BLS and First Aid Certification.
