Schedule: This position is a part of the Nursing Management Team and will regularly work Monday through Friday with rotating on-call and Manager on Duty. This position must be willing to work the floor as needed as well.

Role
We are seeking a dedicated Social Services/Admissions Director to join our team! The Social Services/Admissions Director makes assessment for, plans and advocates for the residents of the community facility so they can reach their highest practical level of physical, mental and psychosocial well-being. The Social Services/Admissions Director will be responsible for pre-admission assessment of referrals as well as discharge planning for all the residents of the Community. Has the ability to communicate effectively and advocate for resident centered/directed care to team members, families, physicians, allied health professionals and the at large community.
Responsibilities
Follows safety policies in performing nursing care, assuring resident safety
Follows infection control guidelines
Attends and participates in facility meetings and in-service training programs as assigned
Initiates emergency measures according to the community facility policy and within standards of nursing practice
Maintains all documentation as required by Federal and State regulations, and the community facility.
Assists with meals in the dining room
Follows and upholds policies and procedures of the community facility.
Follows and understands the community facility's Emergency Preparedness plans
Observes residents rights including, but not limited to, privacy, dignity, etc.
Promotes positive public relations with residents, family members and guests
Coordinates transportation for residents
Completes required forms and documents in accordance with company policy and state and/or federal regulations
Responsible for pre-admission assessment of referrals, coordinates admissions and discharge planning for all the residents
Coordinates and directs the care plan meetings
Facilitates advanced directive decision-making process
Coordinates grievance complaint process
Coordinates and assist with resourcing and scheduling of allied health services
Provides on-going education of residents rights, abuse and neglect prevention to staff
Qualifications
Preference to candidates who are graduates of an accredited school, and currently licensed as RN or LPN in state of which applying.
2 year experience working in a geriatric setting is preferred.
Must possess a valid drivers license
