Peace Healthcare at Dennett Road Manor is a 99 bed skilled nursing facility located in Oakland, Maryland. Our skilled nursing facility has been serving the Maryland community for over 46 years. We’re proud to be a community tradition of outstanding service and compassionate care. We offer 24/7 skilled nursing care, hospice, respite care and many more services to seniors in the tristate area. This is a full-time salaried position. Monday-Friday day shift, with flexible work hours.

QUALIFICATIONS:
Bachelor's degree from accredited school of Social Work or related field is required
Must be an active, licensed certified social worker (LCSW) or a licensed certified social worker-clinical (LCSW-C)
Must possess any certifications/licensures as required by State of employment to practice in long-term care
JOB SUMMARY:
As the Social Worker Director, you will play a key role in addressing the social and emotional needs of our residents, fostering a supportive and engaging community within our long term care facility.
DUTIES AND RESPONSIBILITIES:
Answer inquiries of admission and provide tours to prospective family members/residents
Be actively involved in pre-admission and admissions processes
Orients resident and family to Nursing center, helping both adjust to new environment
Develop a social history on each resident; social assessment and care plan
Prepares and completes social service documentation in timely manner
Participate in resident care plan conference and is active member of care plan team
Refer resident/family member to appropriate social service agencies when facility does not provide services or needs of resident
Chairs Resident and Family Councils to give support to both groups
Serves to reinforce to residents and families Facility's desire to deliver highest quality of care
