Responsible for providing individual and/or group counseling as defined by license. Participate in interdisciplinary treatment planning for all applicable clients. Develop comprehensive diagnostic assessment within the scope of license.
Position Description:
Develop and/or implement individualized client service plans, monitor progress utilizing supportive methods, and determine updates needed in collaboration with the client and team during weekly case conferencing.
Work with offsite support, coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered according to plans developed in conjunction with our partners at the VA.
Deliver on-going services to clients which may include: crisis intervention, advocacy, transportation assistance, budgeting, housing assistance, and referral and linkage to community resources as needed.
Assist in helping clients achieve wellness and autonomy through supportive counseling and facilitating multiple care aspects (care coordination, information sharing, etc.).
Maintain caseload and productivity by maintaining regular contact with clients to monitor and assess progress.
Document all clinical activities to meet specific program, funder and credentialing entity criteria timely and with accuracy.
Ensure compliance with applicable standards, agency policies, and contractual requirements.
Adhere to professional standards as outlined by protocols, rules and regulations.
May require transportation of clients in vehicle.
Other duties as assigned.

Job Requirements:
Bachelor’s degree or higher in Social Work or a related field and at least one of the following licenses: LSW, LPC
2 – 5 years clinical counseling experience preferred
