General Description

The Wellness Coach will work closely with providers, behavioral health therapists and may work with adult and pediatric patients and their families within Evara Health and with community stakeholders to support healthy behaviors and increasing the probability of staying healthy. Wellness Coaches focus on building resiliency, building up strengths and protective factors while educating patients and their families on healthy habits. Wellness Coaches support the work of the patient’s provider and therapist to help patients reach their patient-centered goals. As a member of a care team, Wellness Coaches support Evara Health’s mission, vision and goals and are quality focused to enhance patient-centered care.

Essential Duties & Responsibilities

  • Accepts warm hand offs of patients from medical and behavioral health providers to facilitate integration and coordination of care and provide patient/family education
  • Provides in-person or virtual, individual and group health education and training, using evidence-based programs/curriculums to patients and their families
  • Supports patient and family in identifying self-management goals and develops customized plan with care team including goals and plan of action
  • Develops individualized care plans with patients and their families and provides ongoing monitoring and support
  • Provides case management services for the families with active care plans
  • Tracks patients’ progress and alerts providers to need for enhanced services
  • Coordinates care closely with patients and their families along with other team members to identify patient self-care goals and to assure that medical and behavioral health needs are met
  • Responsible for engaging patients (adults, children and their families) in their care
  • Responsible for coordination of and scheduling of appropriate appointments to include medical, dental, nutrition and behavioral health appointments of patients
  • Responsible for educating patients on other services available at Evara Health and where appropriate offering and scheduling those appointments
  • Participate in CMHI Oversight Committee
  • Prepare and present patient case reviews for the CMHI Oversight Committee
  • Act as a resource for Evara Health and the CMHI stakeholders
  • Accept warm hand off of patients between medical and behavioral health providers to facilitate integration of care
  • Track patients’ progress and alerts providers to need for enhanced services.
  • Responsible for data collection, entry and generation of reports
  • Provide case management services, including referrals to community resources, liaison with medical and mental health providers
  • Coordinate operation of special projects as assigned by supervisor
  • Coordinate care closely with other team members to identify patient self-care goals and to assure that medical and psychosocial needs are met
  • Perform miscellaneous job-related duties, as assigned

Minimum Qualifications

  • Strong organizational, administrative and problem-solving skills, and ability to be flexible and adaptive to change while maintaining a positive attitude
  • Ability to prioritize tasks, work under pressure and complete assignment in a timely manner
  • Ability to effectively present information to others, including other employees, community partners and vendors
  • Ability to seek direction/approval from on essential matters, yet work independently with little onsite supervision, using professional judgment and diplomacy
  • Excellent interpersonal, verbal, and written skills and ability to effectively work with people from diverse backgrounds and be culturally sensitive
  • Conduct oneself in external settings in a way that reflects positively
  • Ability to see how one’s work intersects with that of other departments and stakeholders
  • Make appropriate use of knowledge/ expertise/ connections of other staff
  • Be creative and mature with a “can do”, proactive attitude and an ability to continuously “scan” the environment, identifying and taking advantage of opportunities for improvement
  • Commitment to working directly with low-income persons from diverse backgrounds, in a helpful, supportive manner

Education and Experience

  • Bachelor’s Degree in Social Work, Health or Human Services field, preferred
  • Minimum of three years’ experience with minimum qualifications listed above
