Position Summary

The YYA Navigation Coordinator oversees the Youth Navigation Specialist Team, delivering individualized, trauma-informed support to young adults (16-24) facing housing instability or homelessness. This role emphasizes diversity, equity, and inclusion, utilizing anti-racist principles to address the needs of diverse groups, including BIPOC, LGBTQ+, neurodiverse, and disabled youth. Responsibilities include supervision, coordination, caseload management, direct service provision as needed, and conducting screenings and intake matching with the appropriate Navigation Specialist.

Some Of The Duties Include

  • Serve as point person for YHDP Navigation Team to coordinate activities, provide support, guidance, and direction to Navigation Specialists and liaison to the NAV Collab partners
  • Support the personal and professional development of youth navigators through individualized coaching, trauma-informed supervision, and tailored coaching methods, empowering them to assist peers in achieving housing stability
  • Provide assessment and behavioral health services to transitional-aged youth and their families, developing solution-focused plans, including Medicaid-reimbursable services such as Psychological Diagnostic Evaluation (Assessment), Community Psychiatric Supportive Treatment (CPST), and crisis response
  • Make appropriate referrals and recommendations, advocating and liaising with schools, the justice system, social services, and behavioral health services
  • Contribute to the development and maintenance of clinical records through timely documentation in compliance with licensing and accreditation regulations and standards
  • Maintain accurate documentation to comply with standards and requirements
  • Adhere to best practices, including Trauma-Informed Care, Motivational Interviewing, Housing First, Harm Reduction, and Antiracism

Note: Awareness of how staff behavior impacts individuals with trauma, mental health, and substance use disorders is essential for success in this role.

Qualifications

The ideal possesses a Master’s Degree in Social Work, Counseling, or a related field, along with a valid Ohio LSW or LPC. A minimum of two years' experience in community behavioral health, specifically working with Transition Aged Youth, is required. Prior supervisory experience in this context is preferred. Candidates should demonstrate expertise in care coordination, case management, linkage and referral, and system coordination, with a proven track record of working with diverse special populations and cultural competency. A valid Ohio driver’s license, proof of insurance and reliable transportation, required.
