When we talk about “underresourced” primary care we are talking about situations like this when thousands of hours of uncompensated provider time are the only thing keeping kids out of emergency rooms. And even that isn’t enough. It’s worth noting that uncontrolled childhood asthma is a leading cause of chronic lung disease in adults (which is a major source of our total spending). Guess how many kids have “uncontrolled” asthma in our country…half. Yup. It’s a coin toss. #healthequity #populationhealth

The UK asthma outcomes in the UK are among the worst in Europe. Asthma is almost always an inflammatory airway disease, yet NICE guidance still promotes the use of SABA in mild disease. We know that under use of inhaled corticosteroids and overuse of SABA results in an increase in exacerbations and hospital admissions. It is time we educated our primary care clinicians and patients! Today, on World Asthma Day we are ‘Uncovering asthma misconceptions.’ Common asthma myths include: - Asthma is a childhood disease; individuals will grow out of it as they age - Asthma is infectious - Asthma sufferers should not exercise - Asthma is only controllable with high dose steroids. But asthma occurs at any age, it is not infectious (although viral respiratory infections such as the flu and common cold can trigger attacks) and well-controlled asthma does not prevent people with the condition from exercising or playing sports. Importantly, asthma is most often controllable with low dose inhaled steroids in both children and adults. One of #RACP Evolve’s recommendations for paediatric thoracic medicine asks physicians to not prescribe combination therapy (inhaled corticosteroids with long-acting beta2 agonist) as initial therapy in mild to moderate asthma before a trial of inhaled corticosteroids alone. Murdoch Children’s Research Institute (MCRI) researchers have explored how existing treatments could help ease symptoms for children with wheezing and suppurative lung disease. Dr Melanie Neeland, Professor Prof Sarath Ranganathan and Dr Shivanthan Shanthikumar have co-authored a Medical Journal of Australia piece, explaining how their new research findings may influence the way these conditions are managed. "Preschool asthma or wheezing is a leading cause of hospital admission for young children in Australia...[while] a recent survey in a high risk Indigenous Australian population reported a community prevalence of 12% for the spectrum of suppurative lung conditions in children," they said. "Although the clinical presentations of these two diseases are different, we found that similar inflammatory endotypes are present in the lungs and therefore the same anti-inflammatory treatments could be effective against both."
